Trabajamos en ensayos de prueba de concepto y de mecanismo con terapias dirigidas, con especial énfasis en la señalización celular, las células madre cancerosas y la inmunooncología. Estos incluyen los primeros ensayos en humanos con tratamientos dirigidos, combinaciones racionales de tratamientos selectivos, ensayos basados en biomarcadores y ensayos en poblaciones seleccionadas molecularmente.

Vinculamos la investigación clínica de la Unidad de Investigación en Terapia Molecular del Cáncer (UITM) de la Fundación ”la Caixa” con diferentes áreas de investigación desarrolladas en el VHIO, siguiendo un modelo verdaderamente traslacional. Combinamos biología molecular y modelos de tumores óptimos con farmacología e investigación clínica innovadora en proyectos seleccionados al involucrar a científicos del VHIO en nuestros ensayos (desarrollo de biomarcadores, comprensión profunda de los mecanismos de acción y resistencia).

Este año, nuestro grupo y la UITM del VHIO han continuado liderando el ensayo Basket of Basket (BoB), mediante la obtención de fondos para agregar nuevos módulos a la prueba multimodular. Este estudio académico, respaldado por Cancer Core Europe, integra la preselección molecular, el desarrollo de nuevas pruebas de diagnóstico, como la prueba de ADN circulante, con la evaluación de terapias dirigidas en poblaciones de pacientes que, en combinación con alteraciones moleculares específicas, serán más propensas a beneficiarse de estos tratamientos.

Nuestro Grupo de Desarrollo Inicial de Fármacos y Unidad de Fase I (UITM) continúan posicionando al VHIO como referencia líder en el desarrollo de fármacos y terapias dirigidas en oncología. Testimonio de ello es el número de pacientes que confían en nosotros para recibir atención (499 pacientes inscritos en los estudios de fase I y de cesta en 2019), la cartera de diferentes ensayos disponibles (162 ensayos de fase I, incluidos 22 estudios de cesta en 2019) y lo novedoso de nuestros programas de medicina de precisión y desarrollo de fármacos de inmunoterapia. Esto también se evidencia en nuestro papel de liderazgo en el Clinical Trials Task Force (grupo de trabajo de ensayos clínicos) de Cancer Core Europe.

También hemos fomentado importantes alianzas con la industria farmacéutica, incluida la Iniciativa Partner of Choice de este año con MedImmune, y colaboramos estrechamente con otras organizaciones de investigación clínica y centros académicos de excelencia, así como con empresas dedicadas a promover la medicina y la atención oncológicas personalizadas.

OBJETIVOS ESTRATÉGICOS

  • Desarrollo clínico temprano de las mejores terapias dirigidas en su clase, determinando el programa óptimo y la población de pacientes que probablemente se beneficiarían más de estos medicamentos al participar en ensayos clínicos novedosos.
  • Analizar los tumores de los pacientes en busca de aberraciones moleculares que puedan predecir la eficacia de los agentes dirigidos y permitan seleccionar con mayor precisión el tratamiento más apropiado que coincida con las especificidades de cada paciente con cáncer avanzado.
  • Vincular la investigación clínica de la UITM con los diversos grupos de investigación preclínica y traslacional del VHIO y fomentar colaboraciones eficaces con diferentes socios involucrados en el desarrollo de fármacos y la investigación traslacional (unidades de fase I, centros académicos, consorcios y empresas farmacéuticas).

ASPECTOS DESTACADOS

  • Como referencia líder en el desarrollo de medicamentos a nivel mundial, probamos clínicamente los mejores medicamentos de su clase. Hemos ampliado nuestra experiencia a otros inhibidores de la vía de señalización celular, como inmunoterapéutica e inmunoterapias de segunda generación, así como agentes intratumorales.
  • Hemos llevado a cabo muchos ensayos clínicos con combinaciones doblemente novedosas, incluido el emparejamiento de terapias dirigidas (novedosa/novedosa) y, en inmunooncología, la combinación de inhibidores de puntos de control con quimioterapia, terapias dirigidas u otros agentes inmunomoduladores.
  • En el marco del Programa de Investigación Oncológica Avanzada VHIO-«la Caixa», hemos realizado varios ensayos clínicos con pacientes seleccionados sobre alteraciones moleculares: mutaciones en AKT1, EGFR, PIK3CA, PIK3CB, PTEN, IDH1, ALK, ROS1, BRAF, NRAS , KRAS, FGFR1 y 2, MET, HER2, HER3, RET; amplificaciones en HER2, AKT 1, 2 y 3, FGFR1, MET, NOTCH1-4, reordenamientos de NTRK1-3 ROS1, ALK, BRAF, RSPO2/3, RET y FGFR1-3, y alteración en la expresión proteica de PTEN o sobreexpresión de PDL1, CEA y FAP.
  • Obtención de fondos para un programa destinado a estudiar la resistencia primaria y adquirida a las terapias dirigidas. Este proyecto integra xenoinjertos derivados de pacientes y el análisis de la secuenciación de última generación de múltiples muestras de tejido y ADN tumoral libre circulante. En colaboración con Ana Vivancos, Violeta Serra, Héctor G. Palmer y Joaquín Arribas, del VHIO (investigadores principales de nuestros Grupos de Genómica del Cáncer, Tratamientos Experimentales, Células Madre y Cancer y Factores de Crecimiento, respectivamente), abordamos el factor de crecimiento de fibroblastos y la vía RET.
  • Codesarrollo de pruebas moleculares para el cribado de pacientes (paneles de mutaciones orientadas a enfermedades para plataformas NGS y Nanostring nCounter).
  • Como parte del Programa Integral de Inmunoterapia e Inmunología del Cáncer (CAIMI) de la Fundación VHIO-BBVA, hemos continuado con nuestra línea de trabajo para caracterizar la enfermedad hiperprogresiva con inmunoterapia y colaboramos con la Organización Europea para la Investigación y el Tratamiento del Cáncer (EORTC), para avanzar en nuestro conocimiento de este fenómeno.
  • Seguimos colaborando con Rodrigo Toledo, uno de los investigadores traslacionales del VHIO, para monitorizar el ADNlc de los pacientes que reciben inmunoterapia y caracterizar la evolución clonal de estos pacientes.
  • También en el contexto del programa CAIMI de la Fundación VHIO-BBVA, estamos trabajando con Raquel Pérez-López, investigadora principal de nuestro Grupo de Radiómica, para establecer una firma radiómica que prediga la respuesta a la inmunoterapia.
  • Hemos obtenido fondos para agregar un nuevo módulo al ensayo Basket of Basket (BoB) y explorar la inhibición de FGFR (TAS120) en poblaciones seleccionadas genómicamente.
  • Desde 2019, estamos evaluando el papel de la obtención de imágenes específicas de la dinámica de las células inmunitarias utilizando nuevas estrategias de trazadores, respaldadas por la Iniciativa de medicamentos innovadores (IMI).
  • Hemos comenzado nuestro programa de terapias avanzadas en tumores sólidos, además de implementar nuestro propio programa académico de LIT en colaboración con Alena Gros, investigadora principal del Grupo de Inmunoterapia e Inmunología de Tumores del VHIO, nuestro proyecto de células CAR-T, financiado con una subvención de la Asociación Española contra el Cáncer (AECC) en 2019, y la investigación de los linfocitos citolíticos naturales, desarrollada en colaboración con colegas de la Clínica Universitaria de Navarra, además de otras terapias basadas en células.
